Measure it and set the next 90
Compare against your own week 1, not against an ideal.
About 30 minutes this week
The practice
Ask the same person from week 1 the same question, in the same words. Compare their answer against what you wrote down. Then choose one thing to carry into the next 90 days.
Short read
The change you are looking for is not enthusiasm; it is a lowered threshold. The signal is smaller things arriving earlier — the near-miss mentioned in passing, the “probably nothing but” that turns out to be something. If the answer at week 12 is materially the same as week 1, that is information, not failure: it usually means the loop-closing from week 6 lapsed, which is the most commonly abandoned practice in the program. Ninety days changes a pattern. It does not finish one.

Reflection
What is arriving earlier now than it did three months ago?
Follow your organization's own reporting policy. This program is about the conditions around reporting, not a substitute for it — it is organizational and communication content, not clinical guidance.
