Safety Leadership — The 90-Day ProgramPhase: Embed · Week 11 of 12

When they speak up and they're wrong

This is the week the whole thing is decided.

About 15 minutes this week

The practice

The next time someone raises something that turns out to be a false alarm, thank them for raising it as visibly as you would have if they had been right.

Short read

Every culture of candour is tested at exactly this point. If speaking up is rewarded only when correct, people learn to raise concerns only when certain — and certainty arrives late, usually after the moment when the concern was useful. What you are protecting is the behaviour, not the accuracy. This is genuinely uncomfortable, because a false alarm costs real time and there is a natural pull toward a mild correction that signals “check first next time.” That mild correction is the most expensive sentence in this program. One of them undoes about six weeks of work.

Reflection

Recall the last false alarm on your team. What did people learn from how it was handled?

Follow your organization's own reporting policy. This program is about the conditions around reporting, not a substitute for it — it is organizational and communication content, not clinical guidance.
