The practice
Sit in on one handover as an observer. Do not contribute. Note every point where something was softened, abbreviated, or left out because of time or social friction.
Short read
Handover is a compression event. Under time pressure, uncertainty gets rounded to certainty, hedges get dropped, and the item someone was not sure about — the one worth flagging — is the first casualty, because raising it invites questions nobody has time for. Small structural changes help disproportionately here: an explicit slot for “what I'm not sure about” gives uncertainty somewhere to live, which is otherwise the hardest thing to say out loud in a hurry.
