Take yourself out of it
If it only works when you are in the room, you have not built anything.
About 30 minutes this week
The practice
Choose one thing from weeks 5–8 and make it a standing part of a routine that runs without you — a fixed agenda item, a handover field, a recurring prompt owned by someone else.
Short read
Culture built on a leader's personal attentiveness has a single point of failure: the leader. It survives your annual leave badly and your departure not at all. Turning a behaviour into a routine feels like a downgrade — less personal, more mechanical — but the opposite is true: routines are what let a behaviour outlive the enthusiasm of the person who started it. Pick one. Making four things routine at once produces four things nobody does.
Handover Design Sheet
Which behaviour, which routine, who owns it, what triggers it.

Reflection
If you were away for a month, which of these would still be happening?
Follow your organization's own reporting policy. This program is about the conditions around reporting, not a substitute for it — it is organizational and communication content, not clinical guidance.
