Safety Leadership — The 90-Day ProgramPhase: Invite · Week 6 of 12

Respond in three moves

The response is the whole mechanism. Everything else is preparation.

About 20 minutes this week

The practice

When anyone raises anything this week, use exactly three moves in order: thank, ask, close the loop. Thank them for raising it specifically. Ask one clarifying question. Tell them afterwards what happened as a result — even if the answer is nothing.

Short read

The third move is the one almost everyone skips, and it is the one that determines whether it happens again. When someone raises something and never hears what came of it, they conclude the report vanished — and the next signal costs more to send. Closing the loop is not a courtesy; it is the receipt that makes the system feel real. Note that “we looked at it and decided not to change anything, here's why” closes the loop perfectly well. Silence does not.

Loop Tracker

Raised by / what / what happened / told them on. Three columns and a date.

Optional depth

Not required to complete the week — read it only if you want more on this.

Reflection

How many loops from the last month are still open?

Follow your organization's own reporting policy. This program is about the conditions around reporting, not a substitute for it — it is organizational and communication content, not clinical guidance.
